Have you ever box colored your hair?
Have you ever had a negative reaction from lightener, color, or any other chemicals?

TO MAKE YOU AWARE:

1.Chemical services can have varying results based on your individual hair.

2.Its really important to make your stylist aware of any and all other processes  you have used on your hair.

3.There is a risk of your jewelry (earings necklaces) being damaged or lost during your service. I recommend removing them before your service.

4. Chemical Processes may cause damage to the integrity of your hair. There are products and services recommended to improve and maintain the health of your hair.

I have made my stylist aware of all past processes I have used on my hair to assist in their assessment of my hairs current status and the process I need to achieve the desired result. I realize that this is important information and any information I withhold regarding previous chemical processes will increase chances of damage, disatisfaction, and the potential for unpredictable chemical reactions. I am aware that my stylist is a licensed professional and is trained to perform their absolute best to create the results that I want. I understand that it may take multiple appointments to get to my desired hair goal. I will not hold Sensible Beauty liable if the process has unexpected or undesired results. By checking "I AGREE" below, I fully understand and agree to these terms and would like to continue with my services.
SERVICE RE-DO POLICY: My goal as a stylist is to strive for guest highest level of satisfaction. However if you are unsatisfied or are having challenges with your haircut or haircolor, let me know within seven days of your visit and I am happy to correct the issue with no additional charge. The corrective service must be scheduled within two weeks of the original service. By checking "I AGREE" below, I fully understand and agree to these terms and would like to continue with my services.
FUTURE APPOINTMENT CANCELLATION POLICY: In order to continue providing the best possible scheduling options to by guests, I do require a 24 hour notice should you need to cancel or reschedule your appointment. Should you reschedule within 24 hours of your appointment, you will be charged $50. If you cancel within 24 hours of your appointment, you will be charged $75. If you no call-no show, 100% of the scheduled service will be charged. By checking "I AGREE" below, I fully understand and agree to these terms and would like to continue with my services.
